Survey Utilization*

Surveys can be a very useful tool in program assessment. The following surveys currently are part of the Dental Hygiene Program Assessment Plan. 

Survey Month Notes
Exit survey-Online sample May Administered at end of each Academic year. College ID number used.
Faculty-Online May Administered at end of each Academic year. Use college faculty ID number.
Student Information–Online September Listed as a course requirement in DHY 111. Administered during the first week of the semester. College ID number used.
Six month Alumni-Online October College ID number used.
One & Five year Alumni-Online November College ID number used.
ADA questionnaire November Provided and required by the program's accrediting body.
Employer survey- Online November Mailed to  area dentists and community dental clinics.
Patient Satisfaction October, April, & May Distributed to patients by clinic manager.

*All surveys except the Student Information survey are anonymous. Identification numbers are utilized for data verification purposes only. At this time all online surveys are also offered in paper form with a return envelope provided.
